HopeHealth and Natick Walpole VNA Announce Strategic Partnership

Community VNA has partnered with Natick Walpole VNA, and as of October 1, 2024, Community VNA’s 16-town service area is now part of Natick Walpole VNA’s 54-town coverage area. CVNA is no longer accepting new admissions, and staff have transitioned to Natick Walpole VNA. Referrals to Natick Walpole VNA can now be made by calling (508) 668-1066 or faxing (877) 201-6415.

This transition does not affect hospice and palliative care referrals to HopeHealth.

Our History

Our roots began in 1911 with the establishment of the Attleboro Anti-Tuberculosis Society to curb the spread of a communicable disease that was the leading cause of death during that era. As our commitment to community-based health care has grown, so has the scope of our home health care teams, which now include registered nurses, home care aides, social workers and rehabilitative (physical, occupational and speech) therapists.

Nursing

You or your loved one may require nursing care at home after going through an illness, injury or surgery. Our experienced nurses will partner with you and your health care provider to develop a care plan that best meets your needs. They will monitor your progress, recommend revisions as your condition changes, and closely coordinate transitions of care.

Medical Social Work

You or a loved one may need support to cope with health changes. Our medical social workers are available to assess social needs, offer short-term counseling for patients, advocate for patients, and connect them to community resources to address financial and legal concerns.

Home Health Aides

If you’re receiving services from us, you also may need help with personal care, including bathing, dressing, grooming or moving safely.

Our certified home health aides work under the supervision of our nurses to ensure coordinated care.

Physical Therapy

Increasing your ability to move or walk safely is a priority in your recovery from surgery or injury. Our physical therapists work with you to develop an individualized plan of care and teach you how to improve your strength and balance.

Our goal is for you to achieve the highest level of movement possible. We use a variety of treatment techniques to help you move, reduce pain and prevent disability.

Occupational Therapy

If you’re living with a medical condition or recovering from injury or surgery, you may need occupational therapy to help you with everyday activities. These could include eating, bathing, grooming, dressing and safe mobility.

Our team will help you regain strength and dexterity and rebuild your endurance, balance and stamina. We can also assess your home environment and make recommendations.

Speech and Language Therapy

Our speech therapy team helps you address problems related to speech, language, eating and drinking. These issues could include language and swallowing disorders; cognitive difficulties with thinking, problem-solving and remembering; and slurred speech.

We can also show you how to use an electronic device to translate typed messages into spoken words.

Our specialty programs like telehealth, wound care and LSVT BIG provide top-notch care to you or your loved one in the comfort of home.
