What is Home Health Care?

Wherever you live, that’s where our team will support you. Home health care provides medical services at home to treat a chronic health condition or help you recover from illness, injury or surgery. Medicare, Medicaid and most private insurers cover the home care services we provide at HopeHealth.

How Do I Receive Home Care?

Who Decides Which Services I Need?

Once you’re referred to our services, we'll work with you and your health care provider to understand your goals of care and develop a coordinated plan. Depending on your needs, you could receive care from nurses, therapists, social workers and/or home care aides. Our team is focused on helping you regain your physical wellbeing and independence.
