The Support You Need to Live Independently at Home

If you or a loved one needs short-term nursing or therapy care to recover at home from an illness or injury, HopeHealth Visiting Nurse can help. Our caregivers are dedicated to delivering compassionate, high-quality home care to the communities we serve.

Our services are available to patients in Rhode Island and Massachusetts.

Nursing

After going through an illness, injury or surgery, your loved one may require nursing care at home. Our experienced, trustworthy RNs and LPNs provide care while being respectful of our patients, their families and homes.

Our nurses will partner with you to formulate a plan for your care. They will monitor your progress, recommend revisions as your condition changes, and closely coordinate transitions of care.

Physical Therapy

Our physical therapists will examine you, develop an individualized plan of care and teach you how to prevent or manage your health condition. We use a variety of treatment techniques to help you move, reduce pain and prevent disability. The goal is to achieve the highest level of movement possible.

Occupational Therapy

If you’re recovering at home from injury or surgery, or living with a medical condition, you may need occupational therapy to help you do the things you need and want to do in all aspects of life. These everyday activities could include eating, bathing, grooming, dressing and other self-care activities.

Our occupational therapy team will improve your ability to perform these activities. We also can assess your home environment and make recommendations to help you live there safely.

Speech Therapy

Our speech therapy team addresses problems related to speech, language and swallowing. These issues could include language and swallowing disorders; cognitive difficulties with thinking, problem-solving and remembering; and slurred speech.

Home Care Aides

If you’re receiving nursing or therapy services from HopeHealth Visiting Nurse, you may need help with personal care services, including bathing, dressing and grooming.

Our home health aides are certified by the Department of Health and provide care under the direction of a registered nurse or licensed therapist.

Medical Social Work

Dealing with illness can be challenging for patients and their families. Our medical social workers are available to assess social needs, offer short-term counseling for patients, advocate for patients, and connect them to vital community resources.
