Expand our palliative care services and scope, filling a critical care gap
for those living with serious illness - and those who care for them.

In Rhode Island and Massachusetts, hundreds of thousands of people face progressive, life-changing illnesses.

Every day, they and their families must manage difficult symptoms, numerous medications, the demands of tests and treatments,
and all the stress that comes with it.

We have the expertise to help. Our palliative care programs guide seriously ill patients and their caregivers long before they are ready for
hospice — to help them live their best lives for as long as they are able.

With your support, we will:

Invest in exceptionally skilled people to grow our Advanced Illness Care program. This community-based service brings a nurse
practitioner led team into patients’ homes to help them navigate the challenges and decision points that go along with chronic and often
serious illness.

Develop support and educational resources for caregivers, who disproportionately bear the burden of caring for ill loved ones.
Develop new healthcare and community partnerships to raise community awareness and clinical training about this crucial care —
ensuring that HopeHealth reaches all who need support for serious illness.
