Who decides which home care services I need?
At Visiting Nurse of HopeHealth, teamwork and communication are central to our approach to helping you regain your physical well-being and independence. Once you’re referred to us, we will work with you and your doctor to understand your situation and goals and develop a coordinated plan for your care. Depending on your needs, you could receive care from nurses, therapists, social workers and/or home care aides.
