About Hope Dementia & Alzheimer’s Services
Founded in 1984, Hope Dementia & Alzheimer’s Services has helped thousands of families living with the devastating impacts of Alzheimer’s and related dementias. We provide information, education and support for individuals with these diseases, along with the families and professionals who care for them.
- Support for families: care planning, caregiver education and support groups, respite grants and more.
- Hospice support for patients: multi-dimensional, team-based hospice care tailored for Alzheimer’s and dementia.
- Continuing education for dementia professionals: certificate program and annual conference.

Hope Dementia & Alzheimer’s Services is part of the HopeHealth family of services, which includes hospice care, palliative care, home care, and dementia and Alzheimer’s care.
